How to Access a Family Member’s One Chart | Patient Account
Proxy access allows you to view the health record of a child, dependent adult, or another individual in your care.
There are two ways to complete the request:
- Complete the form online through your One Chart | Patient account. Sign up for an account here if you don't already have one.
- Download a PDF of the form and submit it via email, fax, or mail
Please allow up to four business days for processing once submitted.
Types of Proxy Access
Teens: Authorize Your Teen to Have Their Own One Chart | Patient Account
If you are the legal parent or guardian of a teen (age 13 to 18), use this form to allow your teen to have their own account.

For children under age 19, use this form to request proxy access to their health information.

Adults: Request Access to an Adult’s One Chart | Patient Account
If you are requesting access to the account of a parent, spouse, or other adult family member, complete this form.
Note: Adults can also share access directly through the Share My Record feature in the portal or app. Both individuals must have their own One Chart | Patient accounts to use this feature.

Legal Guardians: Request Access to an Incapacitated Adult’s Record
Use this form if you are the legal guardian or health care agent for an adult patient who cannot make medical decisions for themselves.

Please note: These forms and submission processes are maintained through Nebraska Medicine, the organization that manages the One Chart | Patient platform used by Mary Lanning Healthcare. All proxy requests are processed through their centralized system.
Submitting a Proxy Form
To complete a proxy request online, you must first log in to your One Chart | Patient account. Each “Submit Form” link will direct you to the One Chart login screen—after logging in, the selected proxy form will display automatically.
If you prefer not to complete the request online, you may return the completed proxy form and any required documentation by:

What you need to know
- Parents of children under age 13 have full access
- Between ages 13–18, access is limited to immunizations, allergies, growth charts, and preventive care
- At age 19, access is automatically removed unless the patient grants new authorization
Already have proxy access?
You can view who has access to your One Chart | Patient account by logging in and selecting:
- Your Menu in the upper left corner
- Under Account Settings, click Personalize
- You’ll see a list of who currently has access to your record — and whose records you can view
