Advance Directives

The law requires that everyone being admitted to the hospital have the opportunity to make an “Advance Directive” concerning future medical decisions.

Advance Directives are more commonly known as Living Wills or Power of Attorney for Health Care.

Although you are not required to do so, you may wish to complete an Advance Directive. A Social Services representative will be happy to provide information to help you complete the forms.

If you already have an Advance Directive, please bring a copy with you to the hospital.
