How may I request a copy of my medical records?
Download, print, complete and sign the Authorization to Use and Disclose Protected Health Information forms and return them via fax or mail to Mary Lanning Healthcare, Attention: Release of Information, Health Information Department (Medical Records).

Is there a charge for copies?
There is 25 cents per page copying fee for patients per Nebraska State Statute 71-8404 (http://nebraskalegislature.gov). We will call you to let you know how much the total charge will be once we retrieve your records. You may pay by debit/credit card or bring in a check or cash to the address above. We will mail your records after receiving payment or you may pick them up at our office.
What is pertinent information?
These may include reports in your record, such as radiology, labs, history and physical, and discharge reports.
What are your hours of operation?
We are open 8:00 a.m. to 4:30 p.m. CST, Monday through Friday and closed on national holidays.
When may I expect to receive my records?
Because of the number of requests we receive daily, it may take up to 5-7 working days to fulfill your request.
Will you fax the copies to my home or office?
NO. We cannot fax medical records due to confidentiality issues.
Will you mail the copies or will I have to pick them up?
We can mail the copies to the address you specify on the authorization form, or you may pick them up during our office hours.
How long do you keep medical records?
Adult medical records are kept for 10 years. The records of patients who were 18 years or less at the time of treatment are kept for 7 years beyond the age of maturity.
How may I request copies from my last doctor visit?
If your physician is a Mary Lanning physician you may call Medical Records at 402-460-5717. If your physician is not a Mary Lanning physician contact his office.
Can you explain the charges on my bill?
NO. Please contact Patient Accounts at 402-461-5241. If you have a question on a Mary Lanning physician bill you can contact Medical Office Management at 402-460-5836.
May I obtain a copy of my X-ray films from you?
Yes, please call Health Information at 402-461-5174.
How do I request a copy of my birth certificate?
Mary Lanning Healthcare does not keep birth certificates for those patients born at the hospital. You may request a copy from the Nebraska Department of Health and Human Services, Vital Records at 402-471-2871.
Can my health care provider request my records?
YES, providers can request patient records. Copies of records are provided to your health care providers at no charge.
Can you interpret or explain what is in my medical record?
NO. You will need to contact your physician if you have questions or need help interpreting what you may read in your medical record.
May my spouse/significant other request and receive copies of my medical record?
NO. Another person may not receive copies of your medical record without a written consent form authorizing us to release copies to him or her.
Do you know my blood type?
NO. Your medical records will probably not contain your blood type unless your physician has ordered it, or you needed a blood transfusion for surgery. Those are special labs that have been ordered by your physician.
