Hastings Family Care recently was listed as one of two Nebraska organizations to receive the American Heart Association’s (AHA) Target: Type 2 Diabetes Gold Achievement Award and the 2024 Check. Change. Control. Cholesterol™ Recognition.
The diabetes award was given because Hastings Family Care showed a commitment to improving the quality of care for diabetes patients and lowering cholesterol through improved care for its other patients.

Each year, program participants apply for award recognition by demonstrating how their organization has committed to providing quality outpatient care for people with Type 2 diabetes. In addition to following treatment guidelines, Target: Type 2 Diabetes participants also empower patients with evidence-based information and tools.

This Check. Change. Control initiative aims to improve awareness, detection and management of high cholesterol by educating and empowering consumers, healthcare professionals and patients with evidence-based information and tools, while also improving quality of care for patients through AHA’s quality improvement programs.
